ANCHORAGE- As medical costs continue to rise, one group has a solution that helps keep quality care in Alaska, while improving health care and access for citizens.

The Alaska Primary Care Association released a request for letter of interest to health care providers today. They wanted to know who is interested in transforming their practices to a patient-centered medical home model.

The model was developed to bring about a system-wide health care change to Alaska with three goals in mind: better patient experience, better public health outcomes, and system-wide cost savings.

Alaska Primary is a non-profit dedicated to making healthier communities in Alaska.
